The Wellness Nurse is responsible for the development, coordination, implementation, facilitation and assessment of Wellness Program activities. Wellness programs include, however, are not limited to: Diabetes education, Stress Management, Healthy Eating/Nutrition, Exercise and Parenting Skills.

Implements, promotes and evaluates all Wellness Center programming
Coordinates effective program execution within budget guidelines and with timely and accurate evaluation of all programs including participation, outcome results and overall summary.
Participates in the development and creation of new wellness programming consistent company vision and sustainable health outcomes.
Conducts group education sessions related to chronic disease prevention.
Organizes and oversees fitness or wellness events, programs and information presentations.
Maintains wellness programming schedules, records, and reports
Develops informational materials as needed.
Collects and analyzes data to inform of community needs and project outcomes, prepares reports to meet organization & grant reporting requirements.
Familiarizes and complies with all VNA policies and procedures.
Meets standards of behavior expectations.
Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.
Education: Registered Dietitian or Registered Nurse in the State of Illinois required. Bachelor’s degree in health education, health promotion or related field. Wellness Coach Certification or Group Fitness Instructor Certification preferred. Serve-Safe certification within six months of hire. Certified Diabetes Educator preferred or certification within one year of hire.
Knowledge/Skills: Bilingual in English/Spanish. Proficiency in Microsoft applications.
Wellness program development and facilitation experience required.
Interpersonal skills necessary to develop and maintain effective and appropriate working relationships with Patients and co-workers.
Must be self-motivated and able to work independently.
