Enter all reassessments, psychosocial assessments, and medical evaluations into the Client Track and Access Databases.
Complete new client chart reviews and discharge reviews and submit to respective supervisors for review.
Break down and file closed charts.
Support case managers with administrative tasks such as building new charts, replacing and maintaining chart indexes, and archiving old documentation in accordance with Council on Accreditation standards
Attend all mandatory trainings.
Perform other duties as assigned.
High School diploma or equivalent; Bachelor’s preferred.
Proficient in Microsoft Office and Excel.
Demonstrated excellent oral and written communication skills.
Ability to work independently.
Ability to multi-task.
