The Williams/Colbert Quality Administrator supports the implementation of the Williams and Colbert Consent Decree programs in achieving program goals. The Quality Administrator is responsible for overseeing the transition process from the nursing home to independent living in the community for class members. The Quality Administrator will serve as the liaison between the consent decree administrators at the Division of Mental Health/Department of Aging, housing bridge subsidy administrators, Trilogy outreach teams, and class members. The Quality Administrator will provide member-centered, strengths-based, and trauma-informed services to members with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.
The Quality Administrator will provide at least 50% of services in the community or in nursing home facilities.
The Quality Administrator must be a Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), or must obtain their LCSW/LCPC license within 30 days of being hired.
Oversee the quality of the transition services being provided to class members assigned to the Williams/Colbert transition teams.
Serve as the first point of contact for all new Williams/Colbert class member referrals by completing a comprehensive service plan, steering the direction of the transition services being provided by the transition team.
Collaborate with the interdisciplinary transition team comprised of transition coordinators, nurses and occupational therapists to ensure that the class members are receiving quality transition services.
File reports to the Department of Mental Health/Department on Aging in prescribed format and timeframes which include: routine quality and sentinel indicator reports, and reports of barriers in securing necessary services or supports for class members
Track, monitor, and report on completion dates of required consent decree documentation; i.e. mental health assessments, the securing of stable housing, the transition to the community, and other mental health appointments provided in the community.
Coordinate with service providers, housing entities, and subsidy administrators to ensure and advocate that class member’s housing and mental health needs are addressed and met.
Track all deliverables for class members; i.e., housing, entitlements/benefits, medications, and linkages to report back to the consent decree administrators.
Participate in meetings and conference calls to remain up to date on all consent decree developments, program changes, and new initiatives.
Perform other related duties and/or projects as assigned.
Provide effective supervision and coaching to all assigned team members. Train new employees to provide quality services to clients. Ensure continued education and training for assigned clinical and administrative staff to strengthen services provided.

Minimum two years’ experience working with people with mental health and/or substance abuse diagnoses
Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
Experience working with nursing homes highly preferred
Exceptional customer service skills and ability to interact with a diverse group of internal/external stakeholders
Clinical supervisory experience preferred
