Provide clinical and administrative oversight to a team of clinicians providing intensive, recovery oriented behavioral health services to persons living with serious and persistent mental illness. The focus of the community support team will be to support clients living independently and engaging fully in community life. Team Leader will be an active team member, carrying a caseload, providing clinical interventions, sharing in 24-7 crisis coverage and supervising team member assignments and activities. Complete assessment of potential clients into Permanent Supportive Housing, community living, and/or CST and focus on transition services. Build and maintain CST team and ensure agency performance standards for the team are met and that the team is interfacing with other teams efficiently.
Oversee day-to-day clinical and administrative operations and activities of full-time and part time members of the CST
Coordinate transition of Williams & Colbert members from nursing home to community
Lead Community Support Team meetings and assure team participation in utilization and peer review.
Ensure that care provided integrates all facets of clients’ lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, and housing needs.
Provide a full range of clinical care to a partial caseload of clients all receiving community support services from the team at large. Care will include responsibility for assessment, treatment planning, intensive community based skills training, case management services, and re-authorizations
Accurately complete clinical documentation and service reporting in a timely manner
Participate in clinical supervision; seek individual consultation with supervisor when needed.
Meet and exceed team productivity target
Monitor team staffing needs and hiring and orientation of new staff.
Participate in orientations, all staff meetings and training/ development seminars
Interpret and communicate, at the service delivery level, Center policies and procedures
Submit, in a timely and accurate fashion, schedules, timesheets, mileage and other reports required by the Center’s policies and procedures
Provide clinical and administrative supervision of employees including clinical direction and assistance with treatment planning and service delivery.
Ensure that all clinical activities performed by the team are compliant with standards, policies and procedures of the Community Support Team model, organization and DMH requirements for Colbert/Williams.
Actively monitor individual team member’s targets to assure that the team meets or exceeds team productivity target
Provide performance evaluations for full–time and part -time supervisees building on strengths and identifying ongoing growth/training needs.
Demonstrated knowledge of adult mental illness and treatment modalities including Skills groups, Illness education, recovery stages of change WRAP (wellness recovery action planning), and treatment approaches with dually diagnosed individuals (MI/SUD
Ability to establish rapport, systematically gather data, determine the client’s readiness for treatment and change,
Working knowledge of community resources that enhance recovery
Demonstrate the ability to consistently meet productivity and compliance requirements.
Ability to effectively work as a member of a multidisciplinary team and encourage building of natural community support
Effectively demonstrate initial and continued medical necessity for all care provided
While performing the duties of this job, the employee regularly works in an office setting
Roughly one quarter of the employee’s time will be spent in the community, driving to and from client homes and providing quality assurance services in the home
While in transit, the employee may experience changing road and whether conditions
In supporting clients to live independently, employee may be exposed to varying home conditions
Master’s Degree from an accredited college or university in social work, psychology or a related discipline.
Qualified Mental Health Professional or LPHA
2-3 years experience working with individuals with serious mental illness
1-2 years experience working in non-traditional treatment environments i.e. in-home, residential, community settings
Must have 1 year of demonstrated ability to provide clinical and administrative supervision
Computer proficiency, including knowledge of Word, Excel and Internet skills
