Kaleidoscope Inc, a non-profit, child welfare organization located in Chicago, Illinois is looking to fill the position of Foster Care Nurse- Part-Time, working 25-30 hours a month.
The Foster Care Nurse assists in the oversight and management of the medical needs of children in foster care placement through development, implementation and monitoring of the medical treatment plan of 10 to 20 children/youth working approximately 25-30 hours a month.
Essential Duties and Responsibilities
Demonstrates agency clinical philosophy in all aspects of program activities.
Promptly responds to new foster care referrals by actively participating in pre-placement and placement activities.
Conducts a comprehensive medical file review on all newly admitted clients within 7 business days of admission and assists the Specialist and Foster Parent in identifying medical providers and scheduling necessary appointments
Attend doctors/specialist appointments as needed to understand the care needed and be able to provide appropriate support to children/youth, their caregivers and staff.
Develops, implements and monitors a Medical Treatment Plan for each medically complex client.
Monitor the health and safety needs of assigned clients and to advocate in their best interests; frequency to be determined by client need with a minimum of quarterly in-person visits.
Evaluates the continuing health needs of each assigned client and provides direct nursing intervention or appropriate community referral.
Participates in quarterly Child and Family Team Meetings (CFTMs) on all assigned clients and a monthly consultation with the Specialist and/or Team Supervisor.
Provides training to birth parents, foster parents and staff on child/youth specific medical issues.
Reviews Foster Parent Medication logs for assigned clients to ensure proper medication administration.
Serves as a resource person to staff, clients and foster and biological families on medical issues.
Completes written documentation of all client-related contacts and services provided; documentation to be submitted by monthly deadlines as outlined in agency policy.
Attends monthly FC Program Meetings.
Reports all unusual incidents immediately to FC Director.
Maintains an awareness of community medical resources appropriate to child, youth and family needs, and makes proper use of these resources through referral and follow-up.
Demonstrates a working knowledge of FC Program Plan requirements, COA accreditation standards and related DCFS and policy and procedures.
Completes other duties as assigned by the FC Director.
Expected Hours of Work
Work schedules are based on the needs of the children, youth and families served by the agency and often include nights and weekends. The Foster Care Nurse is expected to work 25-30 hours per month; work schedule to be developed with and approved by Supervisor. A work week is defined as Sunday to Saturday. Must be able to visit families in their homes. Must be able to operate motor vehicle.
Educational Requirements/General Qualifications
Minimum Degree Required: Associates degree in nursing
Degree Preferred: Bachelor's degree
Minimum Years of Experience: At least two years of nursing experience required with experience in working with youth with type 1 Diabetes. Experience working with other children with medical and/or developmental issues.
Licensure: RN license required
Other Requirements: Use of fulling insured, operating automobile and valid drivers license.
