This position collaborates with the Community Support Team (CST) to provide Community Support Team services to individuals assigned to CST in order to decrease crisis episodes and increase community functioning, assisting consumers to achieve recovery goals. This position utilizes his/her own experiences with recovery to support and motivate consumers in their recovery process. This position provides direct services to consumers as outlined in treatment plans in collaboration with the CST and completes all required documentation in accordance with Rule 132, payers, applicable licensing and accreditation bodies, and Sertoma policy. At least one CST staff member is available 24 hours/7 days a week. A minimum of 60% of Community Support Team services are delivered off site in community settings. Staff utilize their privately owned vehicles to provide services in community settings.
POSITION RESPONSIBILITIES: * Through a team approach provide direct services to individuals assigned to the CST, at least 60% off site during times and locations most convenient to the consumer, in accordance with the Individual's Service Plan and complete all required documentation according to Sertoma policy, standards of Rule 132, Attachment B, payers, and accreditation bodies. * Utilize personal recovery experiences to offer peer support to consumers, assisting consumers in developing their own personalized recovery plans for mental, emotional, and physical health. Assist consumers to develop, revise their crisis plans while providing community support services that promote a reduction of crisis episodes and the improvement of community functioning. * Complete assessments such as the CASIG and accompanying documentation required including billing in accordance to standards of Rule 132. * Support the consumer, CST, and other parties identified by the consumer in the treatment plan development process. * Participate in weekly team meetings, maintaining current knowledge and understanding of the treatment, progress, goals, and needs of each consumer on the CST. * Maintain open and effective communication with agency and provider staff, family, and others in response to client's identified needs, client assistance/service needs, treatment plan follow-up, and input on adaptations needed to enhance progress toward goals. Work cooperatively with staff to ensure appropriate access to services as identified during the ITP process. * Maintain relationships with local mental health providers, community psychiatric hospitals, and other support services in order to expand services and enhance supports to individuals served. * Promote the awareness of mental health, both internally and externally. * Function as an advocate ensuring protection of an individual's rights while providing a training element to empower persons with disabilities to understand and protect those rights. * Fulfill other functions on the team, as needed. * Comply with all Sertoma policies and procedures as established.
QUALIFICATIONS: * Certified Recovery Support Specialist or be able to obtain the CRSS credential within one year of employment. * Minimum of one year's experience working with individuals with psychiatric disorders and working knowledge of recovery preferred. * Demonstrates a willingness to utilize personal recovery experience in supporting consumers while demonstrating good judgment. * Experience with Medicaid Rule 132 documentation and billing preferred. * Ability to work in a variety of environments and a willingness to spend at least 60% of services in the community. * Effective communication skills, both written and oral. * Effective organizational skills. * Demonstrated ability to work as a part of a team. * Valid Illinois driver's license and documentation of current auto insurance, with a good driving record and car available. * Must be willing to work evenings/weekend hours as necessary and be on call to respond to emergencies. * Proficiency in the use of computers and working knowledge of Microsoft Word and Outlook applications, with good typing skills.
