Renaissance Social Services, Inc. (RSSI) is an innovative social service agency that provides housing stability services to persons who are homeless and have a disability. We are currently looking for a full-time Outreach Worker, MHP, for our Flexible Housing Pool partnership program. We practice Housing First, and are a Harm Reduction agency.
The Flexible Housing Pool provides intensive supportive services to homeless program participants with mental health, physical health and/or substance use diagnoses who have highly utilized Cook County Health Systems. This project is a partnership with the City of Chicago, Deborah's Place and Thresholds.
Services include outreach, engagement and linkage to partner agencies for Case Management and Housing Location. The Outreach Worker will make efforts to locate and serve each participant identified by Cook County Health Systems. The Outreach Worker would work as part of a dyad of Outreach Workers and would report to RSSI's Clinical Director.
The goal of the program is to help clients attain permanent housing and achieve greater autonomy, develop disability management skills and increase their social support network. This position entails driving to visit clients located in throughout the City of Chicago and 1 week of on-call every month via a work provided cell phone. The Outreach Worker will assess the participant for eligibility for the program. Driving of clients in your personal car is required.
Responsibilities include: assisting homeless by, providing intensive outreach based case management, coordinating services with other service providers, transporting clients to appointments if necessary, and linkage, referral and advocacy with services in the community. In addition, the Outreach Worker provides crises intervention services when needed, and works with clients to increase their skills and improve their self-determination. Maintaining the confidentiality of client information in any form and maintaining accurate and up-to-date records on all clients is done by completing paperwork in Client Track and HMIS in a timely manner.

The ideal candidate should have a Bachelor's Degree in Social Work, Human Services or Counseling. Seeking someone with a background in case-management or outreach. This position requires an individual that is able to work independently and be highly organized. Must be computer literate, have a vehicle with current Illinois insurance and have a valid drivers license.
