Seasonal Outreach Medical Doctor (bi-lingual Spanish/English preferred)
Community Health Partnership of Illinois (CHP), a non-profit organization that provides primary health care to migrant farmworkers, is seeking a dedicated individual who is looking for an opportunity to make a difference in the health of a community in need, focusing on providing care to patients in their communities and at their places of work.
through October 31, 2019, flexible hours
Initiates a comprehensive, ongoing identification of patient needs in order to develop and implement a plan of services and advocacy for that patient.
Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
Collect, record, and maintain patient information, such as medical history, reports, and examination results.
Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
Must be willing and able to work flexible hours, including some early mornings, evenings and weekends to support reaching our target population.
Valid Illinois medical license, Board certification in the area of specialty (FP preferred) and Federal DEA license.
Valid Driver's license and automobile insurance and the desire and flexibility to work in multiple locations.
