Residential Clinical Coordinator is responsible for the overall coordination and operation of one community living site. Accountable for providing to both clients and staff members a positive, professional and safe environment to live, learn, and work. Maintains and assists to assure that all aspects of community living comply with all applicable Federal, State, and Accreditation standards. Coordinator is responsible for completion and distribution of staff/house schedule; scheduling house inspections with outside companies; supervision of community living staff of assigned location. Responsible to develop and implement individual programs to maximize the client’s independent community living skills and inclusion in their community. Provide documentation on assigned clients in accordance with DHS, CARF, or DPH standards, as applicable. Advocate for and maintain rights and privacy of clients that are served.
Responsible for one 24 hour site, a permanent caseload, and additional cases as assigned based on the needs of the program. Provide support for QIDP Managers as well as training new staff as assigned. Supervise support staff as assigned. Perform client case record reviews and advise QIDP Managers on improvements as needed. Prepare a monthly accountability report as assigned. The Residential Clinical Coordinator must be a role model for clients and staff for both permanent and temporary assignments, including providing and participating in direct service as needed.

Bachelor’s Degree in a human service field.
At least one year of experience working directly with individuals with developmental disabilities.
Three to five years of experience as QIDP or QMHP preferred.
One year of supervisory experience preferred.
No lifting restrictions.
Push, pull, reach, stoop/bend over and use step stool.
Perform daily housekeeping tasks and assist clients in independent living skills.
Physically assist clients in situations including, but not limited to: getting in and out of van, bathtub, shower, or bed.
Operate home appliances, including, but not limited to stove, microwave, washing machine, dryer, and vacuum.
Implement behavior protocols and respond to emergency behavioral issues.
The use of personal automobile. Must have and maintain a valid Illinois driver’s license, and the minimum amount of automobile liability insurance, as defined in the Association’s Personnel Policies.
Ability to transport clients in personal automobile.
Must be at least 21 years of age.
Evenings, Weekends, and holiday hours required.
Attendance: Working scheduled hours and flexibility is essential for meeting client and program needs. May be required to work multiple locations.
24 hour on call responsibilities Mon-Fri for assigned community living site for issues and emergencies. Additional on call responsibilities as assigned.
Participation in the weekend on call rotation.
No lifting restrictions that would prohibit assisting in two-person lift or transfer. Must be able to physically assist clients in and out of the van, tub/shower, and bed.
Navigation of stairs.
Complete Agency QIDP training within 6 months of hire.
Maintain 12 CEU’s yearly.
Attend in-service trainings.
