The Patient Navigator will create a welcoming environment for patients and guests of Alivio Medical Center (AMC) and Cook County Health & Hospital Systems (CCHHS). The Patient Navigator shall be highly visible customer serve role that provides a friendly experience when escorting patients, family members, and guest to their designated destination upon arrival and after registration.

Essential Duties and Responsibilities:
Cordially greets patients and guests, ascertains their needs, and directs them accordingly
Provides timely escort services to incoming patients and visitors to designated destinations throughout CCHHS and AMC
Communicates with patients and guests In a sincere friendly manner
Functions as a resource for patients and guests seeking information, assistance, and recommendations.
Assist patients and guests by responding to inquires and providing accurate information about CCHHS and AMC services
Assists patients and guests with checking in at the designated kiosks
Provide follow-up in-home visits
Identifies and escalated priority issues, as needed
Performs other related duties assigned.
H.S. Diploma or GED
One (1) year of healthcare customer service experience
Must be bilingual (Spanish/English). Excellent customer service skills. Ability to establish and maintain effective working relationships with patients, employees, and the public. Ability to communicate with people from various educational levels and backgrounds as well as the ability to speak clearly and concisely. Ability to stand and walk for prolonged periods of time. Ability to follow detailed instructions
Ability to maintain confidential patient information in compliance with the Health Insurance Portability and Accountability Act (HIPPA) Must be highly motivated, self-initiated, and flexible; prepared to work evenings, Saturdays & Sundays.
