Provides coordination and leadership for all Quality Improvement and Performance Improvement activities at NNHSC
Essential Duties and Responsibilities:
Oversee all current Quality Improvement /Performance Improvement activities at NNHSC.
Manage the process of continuous clinical quality improvement including the Quality Improvement staff committee. Directs QI monthly and annual committee meetings. Coordinates, administers and implement actions and processes for Quality Improvement Program using the written QI Plan and using PDSA tools.
Oversee and maintains systems for monitoring and reporting on data and written doccumentation for QI measures, reports, other duties upon request.
In conjunction with CMO, manages quality improvement data generated from the Electronic Health Record system. This data is validated and used for the Q.I. plan.
In conjunction with the Clinical Leadership Team, oversees all aspects of clinical data collection and is actively involved in monthly clinical Dashboards. Will assist in the discussion and dissemination of the monthly clinical Dashboards.
In conjunction with the Clinical Leadership Team, assists in the oversight of clinical workflows using the Electronic Health Record System (EHRS). Promotes standardized written clinical workflow of EHRS workflows. Produces reports as needed.
Provides Quality Improvement oversight with the HIV team and the HIV Quality Improvement Plan and other departments such as Maternal child Health Programs.
In conjunction with the CMO, provides assistance to NNHSC staff in preparation for Joint Commission, other accreditations and site visits.
Acts as a subject matter expert for all new HIPAA guidelines as a base for all Quality Improvement recommendations for NNHSC.
Oversee the development of Quality presentations to Clinical staff, support staff, departmental, senior managers and Board of Directors as requested.
Defines and oversees specific educational objectives for developing community awareness and support for NNHSC Quality Improvement programs and services through consultation with department heads and senior managers.
Provides the Clinical Leadership Team monthly reports on Quality Improvement Activities and clinical Dashboards.
Track patient registry database and key measures and produces reports as needed or requested.
Acts as clinical lead for staff optimization and use of the Electronic Medical Record (EMR). Works with others to ensure optimal use of the EMR and consistent work flows.
Provides the leadership team with quarterly reports of progress and QI initiatives
Develops a curriculum for conducting staff training as appropriate and necessary
Lead member of the Patient Centered Medical Home Committee – PCMH.
Non-Essential Duties and Responsibilities:
Performs other duties as assigned
Assists in health fairs and other Near North special events.
At least 5 years experience in ambulatory health care
Setting. Knowledge and experience of Quality Improvement
Principles and practices. Excellent written and verbal
Communication skills. Proficiency in Windows based
Operating systems. Familiarity with Joint Commission Standards helpful.
