We are now hiring for a Full-Time Bilingual Case Manager 2 to join our Domestic Violence - Midway Center to provide services in the Southeast/Southwest MFS locations.
ESSENTIAL JOB FUNCTIONS:
Responsible for providing brief counseling services to adults victims/survivors of domestic violence.
Complete intakes for new clients.
Assess client's needs and develop service plans with Clients.
Provide individual and group evidence-based and trauma informed services to adults.
Provide psychoeducational adult groups services for adults as indicated by program
Provides outreach and presentations in the community.
Provide case management and complete safety plan with clients as needed.
Provides counseling, advocacy educational and related services.
Provide crisis intervention as needed.
Assess progress and success of intervention through agency approved outcome evaluation tools.
Assesses community conditions affecting clients to identify resource gaps and problems.
Complete and update documentation in a timely manner.
Learn and utilize agency and community resources.
Represent the program and agency in the community.
Meet agency expectation for quantity of direct service and quality assurance purposes.
To comply with agency record-keeping expectations.
Other duties as assigned by supervisor.
Works effectively with diverse staff and service population.
Using intermediate level skills, conducts comprehensive client assessment: works with more complex cases.
Develops a written service plan with the client to meet client need, specifying goals to be achieved and services to be pursued.
Uses expanded knowledge of resources and helps clients utilize them. Works collaboratively with community resources.
Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
Collaborates and participates with other program staff in the planning and implementation of service plans.
Bachelor’s Degree in Social Work or related field required.
Bilingual English/ Spanish required.
One (1) year of case management experience in a social service or healthcare setting experience required
40 DV Training certificate or willingness to take training required.
Experience in working with adults in an office and community setting.
Experience in working with clients and problem solving strategies, implementing Evidence Based practices; experience facilitating groups, ability to work on a team.
Must have excellent verbal and written communication skills.
Ability to work effectively with diverse individuals and groups.
Must work 2 evenings until 8pm.
