Greater Elgin Family Care Center (GEFCC) is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Our mission is to provide quality, affordable health care for all including those without the ability to pay. How You'll Help Patients
Assist patients in initiating and continuing Medication Assisted Treatment.
Engage patients in recovery and support services through harm reduction, motivational interviewing and other evidence-based practices.
Provide individualized recovery coaching to assist patient in their recovery process.
Work with patient care team and patient to develop recovery plans
Assure that all recovery-orientated tasks and activities build upon the individual’s strengths and resiliences.
Demonstrate consistent support to individuals during times of wellness and/or challenging times.
Perform other duties as assigned

What You'll Need:
High school diploma or GED certificate with required
Associates degree is preferred
Current and active CRSS certification
