The primary purpose of this position is to ensure continuity of care and established a formal process for the mother and baby to receive a newborn/PP visit at GEFCC. To heighten hospital staff and provider awareness of GEFCC services.
How You’ll Help Patients
Marketing, promotion of GEFCC services, and outreach.
Provides selected education on postpartum care, newborn care and community services.
Extensive computer knowledge and skills, with the ability to create, maintain, and document in spreadsheets and effectively document in NextGen.
Follow up with early discharge mothers.
Report to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
Facilitates patient referrals to specialty care.
Assists patients with Prescription Assistance Program (PAP).
Assists patients in obtaining prior authorization for diagnostic studies.
Facilitates coordination of care to achieve individual patient clinical measures.
Ensures compliance with GEFCC quality assurance and risk management standards.
Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.

What You’ll Need
High School Diploma or GED Certificate
Graduate of accredited Medical Assistant Program is preferred
Current Medical Assistant Certification is preferred
Must be bi-lingual in Spanish and English
Must have a valid driver’s license and reliable transportation
Flexibility to work early mornings, late evenings, and weekends
