Position Summary: Develop and coordinate employment opportunities for participants.
Consumer case management to include file management, team meetings, case notes, referral.
Development of Individual Service Plan per each individual on case load.
Maintains effective working relationships with referral and networking resources.
Schedules and conducts team meetings with individuals on caseload.
Schedules and conducts job readiness and job placement sessions with assigned individuals. Operates job club. Development/ training of individual job seeking tools.
Maintain an awareness of local job market and opportunities.
Networks with local business to meet their hiring needs, as well as marketing of NRI employment services.
Assists individuals in securing community employment. Screen for employer requirements and assign applicant to suitable opportunities. Arrange employment interviews and attends job fairs.
Educate, promote and provide support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or service plans.
Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
Bachelor’s degree in related field preferred. HS diploma/GED with a minimum of 3 years related work experience
Experience working with adults with intellectual and other developmental disabilities preferred.
Valid Illinois Driver’s License with good driving record.
Two years of related experience working with persons with disabilities preferred.
Excellent communication, interpersonal and organizational skills.
Knowledge of Microsoft Office required.
21 years of age or older
