POSITION DESCRIPTION: Plans, implements, directs and monitors all aspects and services related to Residential Housing and related programs; Supports Quality Improvement and Assurance activities throughout all levels of the agency.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Oversee and manage all of Residential Housing programs, and associated staff.
Oversee a department of managers and line staff providing direct services including case management, residential housing programming, and housing support.
Provide clinical and administrative supervision for the Housing Management Staff and Ryan White Part A and B case managers.
Develop linkage agreements when appropriate with agencies that can support program participants’ efforts towards self-sufficiency and overall program effectiveness.
Develop and monitor yearly budgets for residential and case management programs, and consult on a quarterly basis with the Chief Financial Officer to ensure program costs adhere to established fiscal year budgets and compliance with other fiscal funder regulations
Coordinate a Consumer Advisory Board to provide feedback on program services, effectiveness, engagement strategies, and education program topic.
Serve on Clinical Management PQI Committee, PQI Quality Council, co-chair the Risk Prevention and Management Committee, and oversee activities and progress of other committees including: Training and Supervision, Safety Committee, and the Chart Review Committee
Coordinate and implement relevant PQI activities within the Residential Housing Program, including review and analysis of case record reports and satisfaction surveys, ensuring collection of client and program improvement outcome data and meeting contractual obligations;
Represent departmental staff concerns, responses, and suggestions on the Quality Council
Ensure data is communicated to all departmental staff and that staff feedback is similarly communicated to Quality Council
Provide guidance to staff overseeing and Implementing COA’s Governance Standard, Human Resources, and Administrative and Service Environment
Oversee and coordinate departmental adherence to COA Standard: Supported Community Living Services
Design and implement an annual residential housing satisfaction survey.
Coordinate data collection, input, analysis and reporting of outcomes, and complete quarterly, bi-annual and annual reports, as per agency and funder requirements
Work with Chief Program Officer to ensure that program outcome and indicators are being appropriately met. Create and maintain program outcome reports that can be used to inform program implementation
Monitor the implementation of policy & procedure manuals for Residential Housing Programs to ensure consistent and effective delivery of residential housing and case management services and minimum standards of care. Update and streamline policies and procedures as needed and indicated.
Oversee Quality Assurance activities conducted by departmental staff to ensure that charts are in compliance with the Standard Operating Procedures established by Chicago House, Northeastern Illinois Case Management Cooperative of the AIDS Foundation of Chicago, HOPWA and CDBG Grants.
Conduct Quality Improvement, including assessing and identifying areas for improvement in program delivery, program/client outcomes, and client satisfaction. Design and implement strategies to address program/client needs, and measure their effectiveness.
Oversee and monitor the department’s critical incident occurrences, including reporting any serious behavior changes of clients immediately to the Chief Program Officer. Report any trends and concerns to the agency’s Risk Management Committee.
Work with department staff to complete reports in accordance with funder requirements, and maintain all necessary statistics for report completion
Communicate with federal and local officers to monitor housing and case management projects and ensure that projects are meeting funder expectations and scopes.
Assist and support the Development Department with new housing and case management grant proposals
Assist and support with ensuring compliance and fidelity to COA standards.
Other duties as assigned.
Master’s degree in counseling or social work with clinical licensure is strongly preferred, or equivalent work experience
3-5 years of management experience
3-5 years residential experience preferred
Knowledge of mental health, disabilities, substance use, LGBTQ communities, harm reduction, trauma informed care, and the criminal justice system strongly preferred
Knowledge of HIV/AIDS
Demonstrated excellent oral and written communication skills
Ability to work independently
Ability to multi-task
Proven ability to collaborate successfully across diverse cultural and social communities
