Position Summary: Develops and implements mental health services for individuals with mental illness for the purpose of recovery and community integration.
Completes intake paperwork and provides orientation to individuals entering programming.
Develops, coordinates, implements and monitors individual treatment plans and follow up for identified individuals on caseload.
Facilitates and coordinates community referral and linkage as necessary for individual consumers.
Maintains case files, updates medical exams, releases, etc. and ensures all reports (accident/illness, termination, discharge, etc.) are completed accurately and in a timely manner, in addition to attendance.
Follows State of Illinois guideline in the monitoring of the Medicaid program, including: service delivery and reimbursement, file audits, LPHA consultation, completing mental health assessments, and ensuring Department of Human Services Rule 132 compliance.
Educates, promotes and provides support to individuals in making healthy lifestyle choices, as referenced in the DSP health and safety and health and nutrition training curriculums and/or treatment plans.
Master’s degree in counseling and guidance, social work, rehabilitation counseling, vocational counseling, psychology, pastoral counseling, family therapy
Successful completion of a practicum and/or internship that includes a minimum of 1,000 hours underneath the supervision of a qualified supervisor
Preferred: Bi-Lingual - Spanish
One year experience with individuals having mental illness preferred
Valid and acceptable driving record
Excellent verbal and written skills
Knowledge of Windows XP / MS Office
