The LCSW will - professionally apply social work methods and theory to the diagnosis, prevention, and treatment of psychosocial dysfunction, impairment, or disability, including mental, emotional, and behavioral disorders for families, individuals, groups, and organizations. Screen medical records, conduct interviews and assessments, set treatment goals, and arrange for expediting and coordinating services. Provide comprehensive mental health services in accordance NASW (National Association of Social Workers) code of ethics, with peer consultation and Medical Director over sight, as needed. Work in collaboration with the CHP healthcare team to ensure the delivery of high quality, efficient, and patient-centric care.
Bi-lingual (English/Spanish) and demonstrated cultural competency.
Master’s degree in social work from an accredited institution.
Experience in clinical social work activities, preferred.
Illinois State Licensed Clinical Social Worker License.
