Responsible for providing intake services, rapid HIV testing and counseling, and linkage to HIV care or other ancillary services. Use of intensive outreach strategies to engage People Living with HIV/AIDS (PLWHA) and those high-risk of HIV/STI transmission with services. Assist with coordination and facilitation of Healthy Relationships group interventions. Coordinate and link participants with needed services and community resources. Document all services using program protocols.
High School Diploma or GED required.
Experience working with diverse populations, schools, and community groups.
Experience as a trainer or educator, facilitating health and wellness curriculum preferred.
Reliable and independent transportation.
Flexibility to work evening and weekend hours to meet program operations and needs.
Valid IL driving license and proof of automobile liability insurance preferred.
Bilingual Spanish/English preferred.
