Under the general supervision of the Director of Housing and Senior Services, the Success Resource Lead Case Manager (SRL) will work directly with families to provide case management services to homeless and those experiencing housing instability in the Auburn Gresham community. The case manager helps families develop both short and long-term academic and financial goals, provides financial literacy workshops, and supports the development of school-based student groups to build self-esteem for homeless and near-homeless students. Additionally, the case manager will provide advocacy and referrals to help high-need families access other services available to them in the community through GAGDC's wrap-around services or partner agencies. All services will be provided in partner schools and at the GAGDC office. This position is full-time, 40 hours per week. This is a twelve-month position, FY 2020, with a bi-monthly salary. Continuation of this position and any extra days is subject to extension of grant funds. The position requires a flexible schedule with possible evening and weekend hours.
Provide comprehensive and culturally sensitive case management services to all clients including: intake assessment, benefit assessment, financial goal setting, educational goal setting, money management and financial literacy training, weekly case plan development, progress monitoring, advocacy, and referrals.
Maintain a caseload of 30-40 families, with the goal of serving at least 50 families per year.
Meet with clients weekly or bi-weekly for up to 12 months. For the first six months of client’s participation, at least 80% of follow-up meetings should be held in person rather than over the phone.
Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral.
Input accurate and complete data for all contacts with client into agency database.
Provide all required documentation in a timely manner.
Participate in regular staff meetings, staff trainings, supervisory sessions and other duties as assigned by supervisor.
Adhere to agency policy, procedures and the professional code of ethics.
Perform other related duties as needed
Works closely with GAGDCs Site and Resource Coordinators, Family Engagement and Community Wellness Coordinator, and AG GOLD Parent Advisory Council (PAC)
Parent Engagement; Plan and organize activities and events
Bachelor’s degree in Social Work or related field.
Knowledge of community resources and counseling/social work practices with high-risk populations.
Good documentation skills, and verbal and written communication skills. Ability to establish rapport.
Ability to motivate others towards achieving goals.
Ability to work independently with strong sense of focus, task-oriented, non-judgmental, clear sense of boundaries.
Ability to work in a variety of settings with culturally diverse families and communities and ability to be culturally sensitive and appropriate.
Ability to pass federal background check prior to starting the position
Ability to complete first aid-CPR Certification and Mandated Reporter Certification.
