The Family Support Worker is responsible for supporting the early childhood families in a holistic manner which includes helping them establish a medical home, connect them to the school, resources, conducting home visits and develop goals as defined by each individual family. The FSW will also support enrollment efforts and family engagement activities. FSW will support in the classrooms as advocates for the family and child.
DUTIES AND RESPONSIBILITIES:
Supports the enrollment coordinator on the enrollment process, ensuring that folders are in compliance with Head Start/funder requirements and entering data in agency and funder systems.
Maintains a full caseload. Recruits, retains, and enrolls families.
Refers families to resources, according to needs. Documents referrals and progress towards goals and conducts follow-up with families regarding referrals on a regular basis.
Helps parents establish a medical/dental home to access continuous health services for children and monitor health records in collaboration with program staff.
Monitors all component data on a regular-basis and follows-up with families on missing documentation on a regular-basis, making sure to document all efforts.
Participates in the program self-assessment and community needs process.
Maintains an up-to-date community resource directory in conjunction with the site to assist parents in identifying and accessing local community resources.
Monitors attendance daily and works with staff to follow-up with parents.
Collaborates with program coordinators to ensure all Head Start/funder requirements are met.
Encourages families to attend Passages Early Center workshops and meetings.
Schedules and attends minimum two home visits per year with families.
Support the referral and evaluation process for students with IFSP/IEP’s.
Support the preschool classrooms and staff in the classrooms.
QUALIFICATIONS (SKILLS/KNOWLEDGE AND ABILITIES):
Bachelor’s Degree in Social Work, Human Services, Family Services, Counseling or related field. Must obtain Gateways Family Support Specialist credential level 5.
A minimum of two years of experience working with children and families
Must have a valid Driver’s License, access to a reliable and insured vehicle, and willingness to travel locally.
Experience with multicultural environment
Experience with Reggio Emilia Approach
Bilingual Candidates, preferred
