We currently have an opportunity available for a Permanency Specialist with our Child Welfare Services Department. This position provides and coordinates permanency services for children who will be returned home, adopted or moved into a permanent living arrangement via guardianship. This position uses observation and discretion and takes in consideration the needs of the child and the capability of the parent and families.
Duties may include but are not limited to:
Follow-up with DCFS on status of subsidy applications.
Conduct permanency planning visits for guardianship and adoption cases.
Complete diligent search for adoption and guardianship cases.
Prepare subsidy packets for the adoption cases and guardianship cases.
Identify and follow-up on special court problems specific to adoption and guardianship cases.
Develop resources for children with special needs.
Track internal adoption, guardianship and return home movement.
Provide aftercare services to families that have been reunited with their children.
Assist in resource development for the families i.e., housing, financial assistance for delinquent utility bills, furniture, appliances, school, medical, etc.
Participates in the development of the aftercare plan for children returned home.
Perform other duties as assigned.
Education:ThisPosition requires a Bachelor’s Degree in Social Work, Psychology, Counseling or a related field. This position requires the use of a personal auto, valid driver’s license and motor vehicle report acceptable to the agency’s broker and verification of insurance.
Experience:Minimum two years of experience working in a social work, education or child welfare environment is required. Must have completed adoption training or will complete within one month of hire. Must successfully pass post offer medical physical.
