Erie Family Health Center is looking for a compassionate individual who is interested in community outreach and health education like you to join our Community Health Team! The Community Health Educator works independently and as part of a team to support internal and external community health programs.
Conduct educational appointments with patients, preparing for and facilitating group sessions, providing patient follow-up and presenting educational workshops that focus on nutrition, diabetes, disease prevention and physical activity
Provides training and assistance for staff regarding health education issues
Provides clerical support for providers and clinical service team members including calling, faxing and mailing information to patients and other health care providers
Strong oral communication skills and can communicate effectively with patients, health care providers and community members
Associate’s Degree in Community Health or Health Education (or equivalent of education and directly related experience), Bachelor’s Degree preferred
Strong Interpersonal and oral communication skills
Experience conducting individual or group education presentations
Two or more years working in a community health setting preferred
Demonstrated cultural competency to work in a low income, minority environment preferred
