Responsible for contacting patients and scheduling recall appointments in the various life cycles for an unlimited number of providers, with each schedule item specifying the provider, the time period, the service to be provided, and the site. Responsible for obtaining generated patient lists of various life cycles determined by Medical Director from HIT Department.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Obtains generated patient lists utilizing user-specified parameters defined by supervisor
Calls patients daily to schedule recall appointments
Schedules recall appointments for providers according to guidelines
Allocates available appointment time slots for each provider based on templates for each day of the week including the site and resource for each slot
Enters pertinent data in EMR such as appointment type as needed.
Mail reminder letters for services due (i.e Pap Smears, follow ups or unable to contact letters).
Responsible for providing data as requested by RN Clinical Coordinator
Attends staff meetings as deemed appropriate
Performs other related duties as assigned
Schedules appointment from hospital admissions and ER discharges from Medical Home Network (MHN) portal to appropriate primary care providers.
Assist Care Management team with scheduling patients as needed
Assists Floor Nurses with scheduling Colposcopy appointments or follow up appointments as needed.
Assist Mercy and MacNeal L&D to schedule newborn appointments.
Performs other duties as assigned.
This position requires compliance with all of Alivio’s written standards, including its Standards of Conduct, Joint Commission standards, all policies and procedures and Corporate Compliance requirements. Compliance will be considered as part of the regular performance evaluation.
Manages and supervises staff listed below, including hiring, training, evaluating and managing performance to meet departmental efficiency and performance measures in keeping with the Union Collective Bargaining Agreement (CBA), Personnel Policies and Procedures and prevailing employment laws as appropriate.
RN Site Coordinator
OSHA Category 3 – Involves no regular exposure to blood, body fluids, or tissues, and tasks that involve exposure to blood, body fluids, or tissues are not a condition of employment.
High School Diploma or equivalent
Certification or Licensure:
Medical Assistant and/or medical field background preferred
At least one year experience in customer service and medical assistant position and/or other medical background
Must have good organizational, customer service, clerical and computer skills as well as excellent communication skills. Proficient in medical terminology and immunization schedule.
Ability to work without close supervision in standard work situation. Ability to establish and maintain effective working relationships with those contacted in the course of the work. Bilingual in English and Spanish; oral and written.
