Part time permanent Medical Doctor (bi-lingual Spanish/English preferred)
providing one day of patient care (10 hours) per week
Community Health Partnership of Illinois (CHP), a non-profit organization that provides primary health care to migrant farmworkers, is seeking a dedicated individual who is looking for an opportunity to make a difference in the health of a community in need, focusing on providing care to patients in their communities and at their places of work.
Provide comprehensive medical services in accordance with all CHP clinical policies/procedures/protocols, as well as with State and Federal guidelines to meet generally accepted standards of care. Works in collaboration with the CHP healthcare team to ensure the delivery of high quality, efficient and patient centered care.
Initiates a comprehensive, ongoing identification of patient needs in order to develop and implement a plan of services and advocacy for that patient.
Advise patients and community members concerning diet, activity, hygiene, and disease prevention.
Collect, record, and maintain patient information, such as medical history, reports, and examination results.
Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.
Valid Illinois medical license, Board certification in the area of specialty (FP preferred) and Federal DEA license.
