Community Health Partnership of Illinois (CHP), a non-profit organization that provides primary health care to migrant farmworkers and other medically underserved populations throughout northern and central Illinois, is seeking dedicated individuals who are looking for an opportunity to make a difference in the health of communities in need.
Responsible for coordinating and tracking patient’s referrals to other service providers. Serves as liaison between CHP and community service providers.
Coordinate all referrals made from CHP to other service providers on a daily basis from each site, per the CHP Referral Tracking Policy and Procedure, with exception of those handled by site staff nurses and in house referrals.
Run reports daily to ensure referrals have been coordinated properly.
Run reports on weekly basis and contact patients and/or service providers for high priority referrals.
Ensure appointments are scheduled and kept by contacting patients and service providers.
Help track referrals daily and report to Quality Improvement Manager on a weekly basis to help ensure patients receive the care they need and to identify barriers to such care.
Research and correct invalid or incorrect patient demographic, information.
Verify insurance or self pay information before obtaining specialty appointment and prior authorization.
Maintain current electronic listings of in network specialty providers per Medicaid Managed Care Organizations, commercial insurance, Medicare, and uninsured. Update on a daily basis or as needed.
Minimum Associates Degree or equivalent, such as an LPN license.
Thorough knowledge of GE Centricity and Referral Desktop functionality, including the impact on the patient care.
