Erie Family Health Center is looking for a responsible and focused individual like you to join our Case Management Team! The Case Managers will work with medical providers and nursing staff to identify potential clients, assess, and deliver supportive services to clients.

Responsibilities

  • Outreach, advocacy, assessments, care plan, home visitation, and Cornerstone data entry
  • Collaborates with fellow Case Managers to provide constant floor coverage so as to assure productive presence of a Case Manager throughout the day
  • Displays familiarity and knowledge of the care plan of each client and facilitates referrals and follow-up process
  • Collects participant's surveys as required by grantors
  • Follow the Department of Human Services Maternal and Child code requirements
  • Mandated reporter to the Department of Children and Family Services

Requirements

  • Bachelor's or Master's Degree required in (social work, child development, community health, or related field)
  • Minimum of two years' work experience in health, social or community services preferred
  • Bilingual (English/Spanish) required (May be required to translate for non-English/Spanish only speaking patients and/or guests.)
  • Cultural competency to work in a low income, minority environment
  • Computer skills and time management skills required
  • Ability to maintain confidentiality and trust of clients
