Performs initial assessment and evaluation of new patients.
Responsible for the content of education and counseling programs for both individual patients or groups of patients and serves as a liaison to community, individuals and groups as appropriate.
Interprets to professionals and lay groups the psychosocial needs of the patient by identifying psychosocial disorders.
Provides short term counseling and crisis intervention as necessary.
Acts as a resource to other VNA providers. Collaborates with psychiatrist, physicians, case managers and other providers to develop and implement treatment and intervention plans.
Completion and maintenance of clinical records in accordance with quality assurance standards.
Prepares psychosocial educational materials as needed or required.
Coordinates with other organizational entities involved in mental health services.
Provides in-service presentations to staff as needed or required.
Assumes responsibility for own professional development by attending continuing education, professional workshops and inservice meetings. Completes a minimum of 30 hours of continuing education every two years to maintain clinical licensure in the state of Illinois.
May be responsible for supervision of master’s level interns.
Participates in quality assurance activities of the organization.
Familiarizes and complies with all VNA policies and procedures.
Meets standards of behavior expectations.
Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.

Master’s Degree in social work or counseling and licensed to practice in the state of Illinois
Three or more year of relevant experience. Previous experience in community health preferred.
CADC in addition to LCPC or LCSW preferred.
Bi-lingual in Spanish/English as follows: Reading - Preferred. Written – Preferred. Speaking – Preferred. Must be able to communicate verbally

Valid driver’s license and automobile insurance.
