Coordinates follow-up care, provides non-clinical education, and assesses compliance of patients with follow-up referrals for VNA Health Care internal services and external specialty services. Clinical experience required for this position. Tracks and documents information about effectiveness and outcomes of care to patients presenting for medical/nursing based services at the VNA Health Center Clinic sites. Assists in the call center, providing follow-up patient instructions via telephone. Clinic hours/varies: 2 nights 12-7p or 8-7p a week and 2 Saturdays a month other scheduled days 8a-4p.
Answers and responds to incoming patient calls and triages as appropriate.
Ensure phone calls and requests for information are handled promptly and courteously.
Provides patient education materials to promote patient compliance.
Assists in tasks associated with adherence to prescription assistance programs, medications & lab work support etc. in person or via call center
Screens and schedules appointments for Population Health Programs such as Mammography, House Calls, and other internal programs as directed and in accordance with grant directives.
Reviews, prepares and scans all records prior to the appointments and documents per policy.
Communicates effectively with providers, patients, and family members.
Works within the interdisciplinary team as well as external partners such as tele-radiology, hospital systems, and community facilities to coordinate appointments and services.
Makes reminder calls for appointments for programs as directed.
Performs file maintenance and enter data in electronic medical record or thru other reporting systems such as Cornerstone and Careficent, including scanning of documents.
Tracks and documents information about effectiveness of care and outcomes of referrals.
Completion and maintenance of referral records in accordance with quality assurance standards.
Organizes and coordinates the flow, tracking and filing of patient related documents and correspondence including the movement and distribution of faxes, copies, letters files, reports, medical orders etc. and supplies as instructed.
Ability to work all Health Center hours as directed by the Director of Population Health Services.
Notifies the appropriate provider and/or Director of Population Health Services of problems or issues with coordination of care.
High school diploma required. Some college preferred.
Bi-lingual in Spanish/English as follows: Reading - Required. Written - Required. Speaking - Required.
Effective organization and communication skills, including neat and legible hand writing/printing.
Community health experience preferred.
Maintains current CPR certification.
Excellent computer skills required. Good working knowledge of Microsoft Office software, especially Word and Excel.

Essential Functions/Physical Requirements:
Clinical Experience as CNA, MA or other related experience.

Manual dexterity sufficient to perform nursing procedures and operate a computer.

Ability to frequently lift up to 50 lbs.
