POSITION DESCRIPTION: The primary role of the Supportive Living is to provide clinical management and leadership the Supportive Living Program located in West Town. In this role, the Supportive Living Program Manager will work to ensure quality service delivery to program participants while meeting programmatic scopes required by funders, and oversee all case managers and support staff that work within the program.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Hire, train and supervise staff and client activities at the programs. Monitor and oversee completion of performance evaluation of staff and ensure provision of quality services. Wherever appropriate, coordinate supervision of Ryan White Grant supported staff with the Ryan White Supervisor.
Provide ongoing clinical supervision and support to staff, in concert with Director of Residential Housing, including case consultation, crisis management, oversight of critical incident reports and behavior contract development, and clinical staffing.
Assess the needs of service recipients; the resources available to meet those needs; and the legal and policy requirements governing service delivery.
Oversee and monitor client activities and resident case plans, and provide clinical direction and intervention as indicated.
Oversee coordination of substance use services within residential housing programs
Conduct Quality Assurance activities with client charts to ensure they are in compliance with the Standard Operating Procedures established by Chicago House, and HOPWA Grants
Oversee Program Quality Management activities in accordance with CDPH, as well as annual program dashboards, client satisfaction survey, and logic models (as required).
Conduct intake interviews and facilitate client admissions.
Oversee the distribution and collection of client satisfaction surveys, work with Director to implement program policies and procedures to respond to client feedback
Conduct and oversee quarterly program chart audits and site visits
Develop and maintain effective working relationships with other service providers, networks and outside consultants.
Work with Chief Program Officer and Director of Residential Housing to ensure that program outcome and indicators are being appropriately met. Create and maintain program outcome reports that can be used to inform program design and changes.
Adhere to the Blended Management Philosophy and Approach to housing, and work collaboratively with Chicago House Property Management to ensure stability of housing for clients residing in the program.
Submit monthly/quarterly/annual reports and other correspondences necessary to report on activities and program participants’ progress.
Collaborate with Development Department on grant/contract reporting.
Work with Director of Residential Housing around the oversight and monitoring of the budget for the program
Provide on-site supervision for Master’s Degree Program interns
Attend staff and program-specific meetings, as required
Participate in on call rotation.

Familiarity with HIV disease; Experience in residential services required
Bachelor’s degree in social work and/or counseling required; Master’s degree with eligibility for LCSW or LCPC licensure preferred
Must participate in on-call rotation which includes remaining on call during evenings, weekends, and some holidays.
Familiarity with HIV/AIDS, Domestic Violence, Family Systems, Substance Use, Mental
Health, and LGBTQ Community preferred.

Excellent organization, documentation, and computer skills
Demonstrated excellent oral and written communication skills
Ability to work flexible schedule
Valid driver’s license and car is preferred
