Under the direction of the Executive Director, the Case Manager delivers high quality case management services to the homeless community and to those individuals and families in imminent danger of becoming homeless in Chicago.
Essential Duties and Responsibilities
Meet with clients and perform an initial psycho-social assessment of needs to determine eligibility for Southwest Chicago Homeless Services programming.
Create an individual assessment plan including long and short term goals in collaboration with the client.
Meet at least monthly or more often as needed with all active clients enrolled in case management to monitor client’s progress and needs and set short term achievable goals.
Provide crisis intervention as necessary.
Provide supportive counseling and referrals for securing identification, educational and employment resources and housing.
Provide referrals for medical/psychological/psychiatric treatment and assist clients with applications for disability and other services.
Assist clients in application for permanent or transitional housing for which they are eligible.
Refer clients to needed services to assist in retaining housing and employment.
Enter and track participants in the All Chicago HMIS.
Maintain confidential case files on all clients and provide summary data as required by administration and funders.
Participate in staff meetings discussing client issues as well as office administrative issues.
Other duties as assigned.
2 years experience as a Case Manager.
Bachelor's Degree in Social Services related field
Excellent written and oral communication skills
Previous development, marketing and event planning experience a plus
