Case Management/Social Services, Program Management
The Daniel F. and Ada L. Rice Child & Family Center provides a therapeutic setting for children with severe mental health and behavior challenges. This therapeutic residential center serves youth ages 6 to 14 with a focus on helping children and families stabilize, gain skills, and work toward a goal of transitioning to a home environment. Our treatment philosophy and approach is multi-systemic and draws on the ARC framework, Collaborative Problem Solving, and the latest neurobiological research.
As the Placement & Transition Manager, you will oversee the team of clinical staff who provide case management, assessment, and treatment planning for youth at the Rice Center, with a goal of safely transitioning children into less restrictive environments, and ultimately, returning home.
Placement & Transition Manager Responsibilities:
Provide direct supervision and regular client staffing for a team of Clinical Case Managers.
Ensure the provision of comprehensive case management services, including needs assessment, service planning, case coordination, linkages with community resources, monitoring progress toward client(s) goals and objectives, and recommendations for ongoing services when necessary.
Complete placement documentation as required by internal and external stakeholders.
Attend clinical and transitional staffing meetings.
Maintain and track program and client data.
Collaborate with program leadership to evaluate existing programs and make recommendations for program growth and development.
Maintain current knowledge of Medicaid & MCO requirements, DCFS Transitional Protocol, and Rice Treatment Model.
Work closely with outside providers, including DCFS, Juvenile Court, and other child welfare agencies to ensure continuity of care for clients, and to ensure that Performance Standards are being met.
Attend court and treatment team meetings in the absence of a team member, as needed.
Provide on-call coverage by phone for after-hours emergency needs.
Master’s degree in Social Work, Counseling, or a closely related clinical field.
At least five years of related experience with at least two years case management experience in human service field.
At least three years of supervisory, management, or administrative experience.
