Primary Tasks and Responsibilities:
Engage guests of Streets-to-Home Initiative (STHI) Permanent Supportive Housing Program, and walk-in guests; meet guest needs via outreach, intake, assessment, goal-setting, referral, and follow-up.
Work one-on-one in an ongoing case management relationship with STHI residents.
Plan and run monthly groups and/or activities, and do home visits for the STHI Program.
Create a positive, welcoming, safe atmosphere for guests, particularly by creating a trauma-informed environment and using a harm reduction philosophy.
Become trained in the use of FranStats, HMIS, and Coordinated Entry System, and document all client encounters to agency standard.
Develop collaborative relationships with Franciscan Outreach staff/interns and staff of partner agencies to coordinate care.
Participate in group and/or individual supervision with Director of Case Management, as well as staff meetings and training opportunities.
Various relevant tasks, such as researching community resources and clinical information, attending provider groups, interacting with partner agencies, and any other related duties as assigned.
40 hour work week, Monday through Friday.
Applicant Strengths and Qualities:
Strong interest in and ability to work closely and respectfully with guests who have experienced homelessness and may be dealing with mental health symptoms, substance use disorders, trauma, physical disabilities and health conditions, etc.
Ability to communicate effectively and respectfully with guests, staff, interns, partner organizations, volunteers, etc.
Self-directed and comfortable working without constant direct supervision, and with a team.
Work from a harm reduction perspective and provide trauma-informed care.
Experience using Motivational Interviewing.
Excellent computer skills, including email, internet, and Microsoft Office.
Master’s Degree in Social Work preferred, Bachelor’s Degree required.
