The LCSW/LCPC at NNHSC provides a range of client-centered services that ensure timely and coordinated access to primary medical care, medications, and other support services, including treatment adherence, mental health, and substance abuse for individuals who are HIV-positive and/or have other chronic diseases.
Provide individual patients with the psychosocial support needed to cope with psychosocial stressors and/or medical issues.
Provide integrated care coordination by assisting medical providers by providing supportive social work and therapeutic interventions as required.
Assist patient in identifying/obtaining medical resources. Help patients plan for their home needs by assisting the patient with arranging at-home services.
Conduct screenings, mental health assessments, treatment plans, ongoing interventions, case management, education, treatment adherence, and referrals to referred NNHSC/EIS patients.
Advocate for patients by accessing community services and providing appropriate referrals.
Maintain a system of referral and follow-up to maximize communication with internal and external service providers to enhance optimal patient care and follow-up.
Currently licensed in the State of Illinois as a Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC). Experienced in the field of social work. Must have reliable transportation.
