Aspire is currently seeking a Mental Health Professional to develop and implement mental health services for individuals with mental illness for the purpose of recovery and community integration.
Complete intake paperwork and provide orientation to individuals entering programming.
Develop, coordinate, implement and monitor individual treatment plans.
Facilitate and coordinate community referral and linkage as necessary.
Maintain case files, update medical exams, releases, etc. and ensure all reports (accident/illness, termination, discharge, attendance etc.) are completed accurately and in a timely manner.
Follow State of Illinois guidelines in the monitoring of the Medicaid program, including service delivery and reimbursement, file audits, LPHA consultation and ensuring Department of Human Services Rule 132 & Rule 140 compliance.
Identify and utilize effective advocacy strategies to overcome the diverse challenges facing individuals, i.e., human rights, legal, administrative and financial.
Provide crisis intervention and counseling as needed.
Respond to medical, mental health and behavioral emergencies as necessary.
Run community and/or case management groups.
Assist with development of community resources/community integration.
Provide support in consumers’ natural environment, i.e. consumers’ homes, consumers’ local communities, etc.
Attend and participate in SOARR Department meetings, including check-ins, team meetings and individual & group supervision.
Cooperate and comply with quality improvement initiatives and protocols.
Perform other duties as assigned by supervisor.
Five years of supervised experience in mental health or human services
Spanish speaking preferred.
Substance Abuse knowledge preferred.
Valid and acceptable driving record.
Knowledge of Windows XP/Microsoft Office
23 years of age or older.
