Erie Family Health Centers is looking for an individual like you to joining our Lending Hands for Life Team! The LHL Medical Case Manager provides medical case management services to HIV+ clients at Erie Family Health Center (EFHC).
Works with providers, LHL team members, and other staff at the Health Center to coordinate the delivery of services to clients, assist with access to care, and facilitate the appropriate use of services
Complete outreach, client advocacy, assessment, care planning, and facilitating adherence to medical care plans
Work regularly with other members of the HIV Team including medical providers. Attend team meetings, medical case manager meetings, CQI meetings, site meetings, and weekly case conferences with each provider
Document in the electronic medical record and state-database for all communication related to medical care
Work on projects and initiatives to improve the quality of case management and quality of the HIV program
Assess medical acuity and make home visits and hospital visits as necessary
Bachelor’s degree required (MSW or related field preferred) or the level of knowledge and experience equivalent to that ordinarily acquired through completion of a Bachelor’s degree in social work or a related field is an acceptable option
Bilingual (English/Spanish) preferred, but not required(May be required to assist or communicate with non-English/Spanish only speaking patients and/or guests.)
Two years of progressive experience in a health care delivery organization or health care association
Cultural competency to work in a low income, minority environment
Familiarity with issues affecting individuals living with HIV
Verbal and written communication skills
Computer skills desired
