South Side Help Center’s mission to serve people and strengthen communities is done by providing direct prevention and intervention services designed to enhance the quality of life for individuals, families and community. With a compassion for people, commitment to service, motivation to affect change, cultural awareness and knowledge about your role and responsibilities as a Division of Rehabilitation Services Case Manager (DRS CM) you will help to fulfill this mission by providing a full range of intensive client-centered services focused on improving health outcomes while promoting and supporting client’s independence and self-sufficiency.
As a certified DRS Case Manager you’ll provide intensive client- centered home base case management services, which includes coordination of in-home care and daily living assistance for HIV/AIDS disabled clients. You will work with the client’s medical provider and in-home service provider to ensure client’s ability to maintain care in the community where they reside. The DRS case manager will provide treatment adherence counseling and assist the client in connecting to other core services.
As the DRS CM you can also contribute to the success of South Side Help Center by lending a hand when needed, showing flexibility, paying attention to details, adapting to changes when needed and thinking of your role as a part of the larger SSHC team rather than exclusive.
