The Homeless Prevention Specialist will receive calls from Chicago’s 311 Homelessness Prevention Call Center, gather eligibility documentation, and provide financial and other resources to Chicagoans experiencing housing instability in order to ensure they are able to maintain their housing stability.
Job duties include but are not limited to:
Working within Chicago’s Homeless Management Information System, HMIS, to process referrals,
Managing State Homelessness Prevention and All Chicago Emergency Funds while providing additional homelessness prevention resources for community members in need,
Inform callers about qualifications for homelessness prevention support,
Gathering documents to prove eligibility for funding,
Maintain current information about referral resources for callers,
Tracking all services provided, including tracking the status of referrals,
Follow up with referrals at 3 months and 12 months,
Submit reports to Homeless Prevention program manager and Renaissance supervisor on a monthly basis.
Track calls and services provided and create internal and external reports.
Provide linkage referral and advocacy for referrals with other community providers.
Additional duties not listed may be required depending upon the need of clients and residents and at the discretion of your supervisor.
Must have the ability to relate appropriately and effectively with others and have excellent phone manners. Must be able to relate professionally with other staff and personnel at other community agencies; knowledge of and access to community resources, instructional skills to present information; organizational skills and ability to document services are a must; knowledge of and willingness to match programs and services appropriate for callers; ability to establish and maintain effective working relationships; ability to work independently and as a member of an interdisciplinary team.
Bilingual Spanish/English required
Associate Degree or some college and two years of recent experience in human services.
