SASS/MCR Post Crisis Specialist
Provide comprehensive mental health assessments to community based clients residing in the SASS service area who have received crisis screenings by SASS workers. These assessments will occur at the hospital, in the home, or when appropriate, in the office.
Collaborate with SASS clients and their families to create treatment plans reflecting client’s preference for care and demonstrating medical necessity.
Provide stabilization and support services to SASS clients and their parents during their 90 day eligibility period. This may include but are not limited to the following:
Provide initial screenings and mental health assessments that identify service needs
Recommend options for care.
Share assessment findings with clients and their families.
Involve client and their families in the development of treatment plan goals, objectives and interventions that address their assessed behavioral health needs.
Provide individual, family and group therapy services.
Provide case management services to assist clients and their families with access to needed benefits and services
Provide coordinated care by initiating or participating in client centered consultation with other internal and external providers involved in care or services.
Reassess and review treatment progress with clients and their families at proscribed intervals.
Assist in establishing effective communication between the Center, other social agencies serving clients, and the community
Serve as the contact for the C&A Community Services Coordinator/ICG coordinator and C&A Outpatient Coordinator for appropriate referrals.
Facilitate linkage to other services provided at KYC based on assessed need.
Participate in consultation, as needed, with other agency professionals or with paid agency consultants.
Provide on-call crisis assessment services and clinical back-up (1-2 times a month or as needed).
As required by Rule 132, meet face to face with clients to complete and authorize mental health assessments.
Participate in or, when directed by the SASS Coordinator, preside over peer utilization review.
Assume responsibility for administrative duties as assigned, such as:
Case review for closing
Interpret and communicate, at the service delivery level, Center policies and procedures.
Submit, in a timely and accurate fashion determined by supervisor, reports and data as required by the Center’s policies and procedures.
Attend SASS department meetings.
Meet agency quality and compliance standard by accurately completing clinical documentation and service reporting in a timely manner.
Meet or exceed service productivity target.
Participate in program evaluation and development of service options that best address client needs.
Participate in Staff training and development seminars.
Work cooperatively with other Center teams and programs to maintain smooth continuity of care when cases are shared across teams.
Become familiar with and utilize of outside resources as needed to provide comprehensive services to clients.
Participate in supervision with Coordinator and in regular performance appraisals as directed.
Provide Intake, Screening and/or Crisis Assessment and Intervention Services
Complete Comprehensive Behavioral Health Evaluations and Assessments
Provide Treatment/Care Planning and Services
Provide Transition (linkage, discharge planning) Services
Bilingual Spanish skills are preferred, but not required
Demonstrate knowledge of child and adolescent development and treatment theory, emotional disorders, and community based systems.
Familiarization with statutes and codes pertaining to minors.
Familiarization with and utilization of outside resources as needed to provide comprehensive services to clients.
Provide Child and Adolescent treatment services within a Wraparound context involving multiple system planning and coordination, intensive family based services, and differential treatment approaches and interventions.
While performing the duties of this job, the employee regularly works in an office setting
Roughly one quarter of the employee’s time will be spent in the community, driving to and from hospital sites to perform crisis screenings and or client homes.
While in transit, the employee may experience changing road and whether conditions
In conducting, screenings and follow ups with the family, an employee may be exposed to varied residential environments based on assigned caseload.
Masters Degree from an accredited College or University in Social Work, Psychology, or closely related discipline meeting the requirements for QMHP under Medicaid Rule 132
Lower level licensure preferred.
One to three years experience working with children and adolescents.
Valid driver’s license, proof of insurance and reliable transportation required.
Excellent driving record.
Computer skills required.
